This site contains the majority of the Premium Tax forms that are required to be filed with the Department. However, certain company types were not included as options for download. A complete packet will be mailed the first week of January 2012 to those foreign company types that do not fall under Property & Casualty, Life & Health, HMO, or Title types. If you do not find the Premium Tax packet for your company and you do not receive a tax packet from the Department by January 15, 2013, please contact Martha Hettenbaugh either by e-mail at Martha.Hettenbaugh@nebraska.gov or calling 402-471-2201.
